This issue was resolved and all completed assignments should appear as such with grades reporting to the gradebook.
**No action is required from instructors or students as all submitted work was preserved. Grade sync queues will clear automatically as external Learning Management Systems (Canvas, Blackboard, etc.) finish processing the accumulated data for integrated courses**

Investigating
Our technical team is actively investigating an issue which is resulting in assignments that were completed by students not reporting to the gradebook and in some cases still showing as "In Progress".
**No grades have been lost.**
Grades are recorded in the background but they will not currently show up in either Achieve or downstream in local Learning Management System's gradebooks.
Students can continue to submit assignments and grades will eventually show up in the gradebook.
We will continue to send updates until grades are reporting as expected.
